Quando Kathy Thorn dà alla luce un bimbo morto, il marito Robert per risparmiarle il devastante dolore lo sostituisce con un neonato orfano, ignaro delle sue origini sataniche. L'orrore comincia il giorno del quinto compleanno di Damien, con il drammatico suicidio della sua tata. Di lì a poco un sacerdote che cerca di mettere in guardia il padre di Damien muore in un bizzarro incidente. Dinanzi a sempre più uccisioni Robert capisce che il figlio altri non è che l'Anticristo incarnato e decide di ucciderlo per sventare una profezia catastrofica.
